Customer Requisition Form

The Partner completes this with the customer before submission, or the customer completes it directly. Every field below is required unless marked Optional. Incomplete forms are the #1 cause of development delays — nothing proceeds to Step 4 (Development) until every required field is confirmed.

1. Business Information

Basic facts about the business the AI Agent will represent. This becomes the AI's understanding of "who it works for."

2. Primary Contact & Authorization

Who Alisons Technology and the Partner should contact for approvals, questions, and the walkthrough session.

3. AI Agent Configuration

How the AI Agent should sound, what it's called, and when it's expected to be working.

Plan prices shown are the White-label Cost per Schedule B — what the Partner pays Alisons Technology. Use the "Open Pricing Calculator for this plan" link above (once a plan is selected), or visit the Pricing Calculator, for a full cost breakdown and to set your own retail price.
4. Call Handling & Routing

Where a call goes when the AI can't (or shouldn't) handle it alone.

5. DID / Phone Number Configuration

The actual phone number the AI Agent will answer on. Get this section right early — number porting is the step most likely to delay a Go-Live date if it's started late.

Only complete the fields below if "Port existing business number" was selected above. Porting typically takes 3–10 business days depending on country and carrier — start this early so it doesn't delay Go-Live.

6. Knowledge Base, FAQs & Policies

Everything the AI needs to actually answer questions correctly, in the business's own words and policies — not generic answers.

7. Appointments & Lead Capture

How bookings actually get made, and what information a lead needs to leave behind on every call.

8. Integrations Required

Any system the AI Agent needs to read from or write to. Leave blank if none — the agent works standalone by default.

9. Compliance & Data Handling

Flags anything that needs written sign-off from Alisons Technology before development starts — see the Compliance rule in the Partner Program Guide.

If "Yes" is selected above, this request requires written compliance confirmation from Alisons Technology before development can begin — flag it to your Account Manager immediately.
10. Tone & Custom Instructions

The AI's personality — how it should sound is often as important to the customer as what it says.

11. Add-Ons & Authorization

Final sign-off. Once this is signed, the order is locked and moves to Step 4 (Development).
